Space time is a four dimensional continuum or a four dimensional world obtained by combining three dimensional spatial co ordinates and time. The idea of space time was first proposed by German Mathematician Herman Minkowski and known as Minkowski space time and explained in detail by Albert Einstein in his theory of General Relativity.
Explanation
Before Albert Einstein, the space and time are considered as separate entities. Speed of light is said to be varying and governed by Maxwell's laws. But Albert Einstein in his famous Einstein E=MC^2 postulated that speed of light is constant everywhere in the universe. For the light to be constant to all the observers, the space and time should be considered as a single unit and thus he proposed a four dimensional continuum called space time. For example, a person travelling to the near speed of light will experience a dilation in his time and space. That means the time slows down for him and length contracts . As he obtains light speed, his time and space contracts and become a single entity.
Space time fabric
Einstein, in his general relativity theorem also said that the space and time are not different single entities but a infinite four dimensional woven sheet extending all the way around the universe. Space time fabric explains the gravitational phenomenon of all the celestial bodies having mass. Objects with greater mass curves the space time more and has more gravity. For example, consider a cloth pinned at four sides of a horizontal hollow wall which we can assume as a space time fabric. Then take stones of 50kg and 10kg which we can assume as sun and earth respectively and drop it onto the cloth. The 50kg stone that is the sun curves the cloth deeper than the 10kg stone that is the earth. So the sun has more gravitational energy than the earth. This concept also explains about black holes and space portal. Now consider putting a stone of one thousand kg onto the cloth which is a black hole. The 1000 kg rock bends the cloth that is space time to such an extent that it protrudes to the other side of the cloth. In the same way, black holes bend the space time to such an extent that it breaks and opens a space portal to the other world, possibly other universe.
